Bioidentical Hormone Replacement Therapy (BHRT) is a natural hormone treatment that uses hormones chemically identical to those your body already makes.
BHRT is commonly used to help women experiencing perimenopause, menopause, or hormone imbalance feel better, regain energy, and restore overall health.

- Fatigue or low energy
- Hot flashes and night sweats
- Mood swings, anxiety, or depression
- Weight gain, especially around the belly
- Brain fog and poor sleep
- Low libido or vaginal dryness
When guided by an expert, personalized BHRT treatment plans can help improve hormone levels, reduce symptoms, and support long-term wellness—naturally and effectively.
With Amanda Spaak

Complete your informed consent form - Your informed consent form is a key first step that lays the foundation for your personalized hormone plan.
Once Amanda reviews your intake forms, she’ll send personalized lab instructions tailored to your needs. Lab work is ordered through Rupa Health, and includes blood work at a nearby Quest Diagnostics location and a saliva cortisol test mailed directly to your home. Additional labs may be recommended if needed, and all testing must be completed through our process to ensure accurate results.
Once labs are back (typically 7-14 days), Amanda will invite you to a 60-minute comprehensive consultation (virtual or in-person in Fort Collins). You'll receive:
- A customized hormone protocol
- Supplement recommendations
- Guidance on filling any prescriptions through our compounding pharmacy.
Complete a short follow-up form and meet with Amanda for a 30-minute check-in to make data-backed adjustments as needed.
Amanda will continue to monitor your progress and make adjustments to ensure your plan remains optimized for your changing needs.
Once your plan is dialed in, you'll rarely need frequent follow-ups. However:
Annual Review (Required for BHRT patients):

- Quick check-ins for questions between appointments.
Amanda Spaak, NP is a Colorado native with 19 years of medical experience and advanced training in Functional Medicine and Bioidentical Hormone Replacement Therapy (BHRT). She is dually certified as a Family Nurse Practitioner and Adult-Gerontological Acute Care Nurse Practitioner. After years in acute care, Amanda saw how often symptoms were treated without addressing root causes, which led her to pursue a more personalized, functional approach to care.
As both a provider and a patient, Amanda understands the impact of hormonal imbalance, chronic stress, fatigue, and inflammation. Functional Medicine helped her uncover answers in her own health journey, and today she empowers patients through education and individualized hormone care. Amanda is trained through The Institute for Functional Medicine, certified by the Functional Medicine Academy, and certified in Hormone Management through the BHRT Academy.

BHRT addresses the root cause of symptoms — hormone imbalance — rather than masking them with one-size-fits-all medications. That’s why it’s one of the most effective tools in functional and integrative medicine for men and women’s health.
Candidates for BHRT often experience symptoms like fatigue, sleep disturbances, hot flashes, low libido, brain fog, mood swings, weight gain, joint pain, vaginal dryness, or issues related to bone health. A consultation with lab testing can help determine if BHRT is suitable for you.
BHRT is tailored based on comprehensive lab tests (saliva or blood) that measure hormone levels. These results, combined with your medical history and symptoms, guide the creation of a personalized treatment plan, which may include compounded medications in forms such as creams, patches, or oral capsules.
Most patients start noticing symptom improvement within 4-6 weeks. However, it can take up to 6-9 months to fully optimize hormone levels. Regular follow-ups and adjustments are crucial during this period to ensure the treatment meets your goals.
As with any medical therapy, there are potential risks. Common side effects may include:
- Temporary adjustment symptoms like mood changes or mild bloating
- Skin irritation at application sites (for creams or patches)
- Rare risks of blood clots or other complications, especially in individuals with a history of certain conditions
Discuss your medical history with your provider to minimize risks and determine the safest approach.
For those seeking non-hormonal options, we can explore natural supplements, lifestyle changes (diet, exercise, stress management), and other therapies tailored to reduce menopause or hormone imbalance symptoms.
Unlike traditional hormone replacement therapy (HRT), which often uses synthetic hormones, BHRT uses bioidentical hormones derived from natural sources. These are specifically compounded to match your body's hormone structure, potentially reducing side effects and improving efficacy.
Yes, BHRT can address weight gain and muscle loss caused by hormone imbalances. For example, an imbalance in testosterone, progesterone, or estrogen levels can contribute to increased belly fat, reduced muscle tone, and metabolic changes. Personalized treatment may improve energy, metabolism, and physical strength.
Yes, many patients switch from hormone pellets to other BHRT forms such as creams, patches, or oral medications. These methods provide more flexibility in dosing and easier adjustments based on follow-up lab results and symptoms.
Saliva testing is preferred because it measures the bioavailable ("free") hormones, which are the active hormones in your body that directly impact symptoms. This method is highly researched and recommended for detecting imbalances early and monitoring treatment progress. It also helps ensure we use the lowest effective dose for your therapy.
If your lab results show balanced hormone levels, transitioning to a transdermal or oral hormone formulation is typically smooth. However, if you're switching due to hormone buildup or imbalance, it may take 6-9 months for your body to stabilize. During this period, we closely monitor your symptoms and adjust your dosage to ensure a safe and effective transition.
Coverage for BHRT varies by provider and plan. While some insurance plans may cover compounded medications or testing, others may not. We recommend contacting your insurance provider or discussing payment options with our team.
